Staphylokokken: S. aureus, Koagulase-negative Staphylokokken ca. 10-30%; postoperative Serien höher Postoperativ, posttraumatisch, Fremdmaterial, Endokarditis, hämatogen.
Gramnegative Enterobacterales: Klebsiella, Proteus, E. coli, Enterobacter ca. 5-20% Nosokomial, otogen, immunsupprimiert, hämatogen.
Pseudomonas aeruginosa insgesamt selten; höher bei chronischer Otitis/nosokomialer Genese Bei Otitis/Mastoiditis, nach Neurochirurgie oder bei Resistenzrisiko gezielt abdecken.
Nocardia spp. meist <5%, aber klinisch wichtig Immunsuppression, Steroide, Transplantation, pulmonaler Fokus; multiple Abszesse möglich.
Mycobacterium tuberculosis / atypische Mykobakterien selten in Mitteleuropa; regional abhängig Immunsuppression, TB-Endemie, chronische Läsion.
Pilze: Aspergillus, Candida, Mucorales selten bei Immunkompetenten; relevant bei Immunsuppression Neutropenie, Transplantation, hämatologische Erkrankung, Diabetes/Mukormykose.
Parasiten: Toxoplasma gondii kein klassischer pyogener Abszess; wichtig bei HIV/AIDS oder anderer schwerer Immunsuppression Ring-enhancing Läsionen bei schwerer zellulärer Immunsuppression.


Bei Patient:innen mit einer Immunsuppression sollte man zusätzlich an Nokardien, Actinomyceten und Pilze ( Cryptococcus neoformans,selten Mucorales,Aspergillus spp., Candida spp.(bei generalisierter Candidose) und an Toxoplasma gondii ) denken.

Scedosporium apiospermum ist nach Beinahe-Ertrinken sowie bei Immunsupprimierten ein möglicher kausaler Erreger. Bei entsprechender Reise- oder Expositionsanamnese sind auch Protozoen (Entamoeba histolytica, Baylisascaris procyonis,) und Parasiten (Schistosoma spp.) sowie Würmer (Echinococcus spp., Taenia solium, Paragonimus spp.) in Betracht zu ziehen [1][3][4].

Das Erregerspektrum wird maßgeblich bestimmt durch den Primärfokus.

Hämatogene Streuung Streptokokken, S. aureus, gramnegative Erreger Endokarditis, Lungenabszess, AV-Malformation, Zyanosevitien; oft multiple Abszesse.
Postoperativ / posttraumatisch S. aureus, Koagulase-negative Staphylokokken, gramnegative Stäbchen, ggf. Pseudomonas Empirisch MRSA- und nosokomiale Gramnegative abdecken.
Immunsuppression Nocardia, Aspergillus, Mucorales, Candida, Toxoplasma, Mykobakterien zusätzlich zu Bakterien Frühzeitig erweiterte Diagnostik, molekulare Tests und gezielte antimykotische/antiparasitäre Therapie prüfen.
Kryptogen häufig Streptokokken/Mischflora; Quelle unklar Zahn-, HNO-, pulmonaler und kardialer Fokus aktiv suchen.
